Facilities Use Form

Avondale United Methodist Church has been at the heart of the Avondale community for over 125 years.  Throughout this rich history, our facilities have hosted a diverse array of religious, cultural, secular, community, and private events.  If your or our organization need a place to come together, we welcome you to complete the form below to let us know about your need.  We are able to accommodate most requests.   

The event space approval process involves three steps:
  1. Complete the form below.  Before filling in the date fields, it is recommended that you check the church calendar to make sure there are no conflicting events.
  2. A church representative will contact you to confirm your request or to obtain further details about your event.
  3. If your event is approved, you will meet with a church representative and complete a building use agreement at a pre-event consultation.

Please note that no alcoholic beverages are to be served or consumed on the premises of Avondale United Methodist Church.
Avondale United Methodist Church is a smoke-free facility.

    Event Information

    This section requests information about your event.

     
  • Event Date - Start*
    Enter the date your event will begin.
     
     
  • Event Start Time*
    This is the official start time of your event. The time you will be allowed access to setup will be arranged with the church office.

    Enter the date your event will end. Leave blank for one-day events.
     
     
  • Event End Time*
    This is the official end time of your event. The time allowed for cleanup will be coordinated through the church office.
     
  • Recurring Event?*
    Is this event a recurring event, i.e., does it happen weekly, monthly, or at some other regular schedule?
     
  • Recurring Event Schedule
    If you indicated that your event is a recurring event, please describe. E.g., meets every Tuesday, meets on the third Wednesday of the month, etc.

    Describe your event. Include the type of event and cost to participants.
     
  • Will outside vendors be used?*
    Outside vendors include chair/table rental companies, caterers, DJs, and florists.

    Furnishing and Configuration Needs

    If use of furnishings is granted, the user accepts responsibility for setting up and taking down tables, chairs, and other furniture; returning furniture to its storage area, and resetting the room for regular church use.
